Case Management is a collection of coordinating, brokering, advocacy, assessment and administrative activities which connects the person, child and/or family with the treatment and support they need.  Case Management is divided into three types of service:  Administrative Case Management, Resource Coordination, and Intensive Case Management.  We serve Western Montgomery and Northern Chester Counties.

Administrative Case Management includes the following           *   Registration of the adult/child into the Mental Health System           *   Assessment of client needs and connection to services, which include                Mental Health Treatment Planning, Crisis Intervention, Outreach and Financial Entitlements

Resource Coordination is a service for adults/children with serious emotional disorders and their families who do not need the intensity and frequency of contacts provided through intensive case management, but do need assistance in accessing and coordinating resources and services.  Services provided include:           *   Assessment/Service Planning           *   Crisis Resolution           *   Support Network Building           *   Identifying and linking with community resources

Intensive Case Management is a service for adults with serious and persistent mental illness, and children with serious emotional disorders and their families.  Intensive Case Management is designed to provide aggressive and creative strategies in order to gain access to resources and services as outlined in the clients treatment plan.  Services provided include:           *   Service Planning           *   Linking clients to mental health treatment           *   Connecting clients to educational and employment opportunities           *   Developing social supports           *   Establishing financial support
